Healthier Communities and Older People Partnership

The West Norfolk Healthier Communities & Older People Partnership aims to fully contribute to improving the health of the population in West Norfolk and reducing health inequalities for the most disadvantaged groups. The Partnership actively engage the Primary Care Trusts and other NHS agencies, working in collaboration with the local authority, voluntary and community sector, business sector and other statutory agencies.
 
This partnership has made a commitment to tackle the following priorities -
- Fewer people under the age of 75 die from Cardio Vascular disease
- Improved Mental Health
- Older people live safe, independent and fulfilling lives
